Martin County Public Health Department - Public Health Nurse & HANDS Family Specialist
The Martin County Health Department is accepting applications for a full-time Public Health Nurse and HANDS Family Specialist. This position will be cross-functional spending approximately 75% of their time working with HANDS families and 25% of their time working with diabetes programming and in the public health clinic. Applicants should possess strong, proactive problem-solving skills and the ability to work well in teams in the collaborative and supportive culture of the Martin County Health Department.
The selected candidate will join our award-winning HANDS Program for families and should enjoy working with families and occasionally keep flexible hours doing family home visits. Since diabetes programming is a new program in the health department, the selected candidate will also have the opportunity to grow in this role and establish a premiere diabetes education, prevention, and management program.
More specific duties, in addition to other duties as assigned, will include:
HANDS Family Specialist:
- Conduct initial home visit following Parent Visitor assessment with first-time parent/s and or families to establish rapport and planning for providing services as qualified by the provider.
- Provide regular planned visits to establish a trusting relationship for voluntary eligible HANDS participant/s to assist first-time parent/s and or families with child development, parenting skills, health services, and other needed resources identified at assessment or during visits.
- Facilitate families in securing prenatal/child health services, referrals, and appointments with other supportive agencies.
- Document case records, and home visit logs accurately on each contact, progress, and service need and complete reports as required.
- Maintain confidentiality of program information obtained from home visit/s and exchanged while communicating with community and support service providers.
- Confer with the Program Coordinator and/or Supervisor in cases involving potential collaboration with other service providers.
Public Health Nursing and Diabetes Program Management:
- Manage referrals to DPP and DSMES programs and coordinate with patients and providers (both internal and external to the health department) for ongoing care and clinical care.
- Coordinate with external partners to deliver diabetes/prediabetes education.
- Support community outreach and awareness programming by performing screening, A1C testing, and any other services surrounding MCHD diabetes/prediabetes.
- Provide DPP and DSMES classes for those with pre-diabetes and diabetes to the general public as a part of public health programming.
- If not already certified, promptly obtain and maintain certifications to instruct on DPP and DSMES course materials.
- Work as a public health nurse in both the health department’s in-house and mobile clinics as needed. These duties may include nursing and WIC assessments, vaccinations, support for women’s health and family planning services, TB skin testing, and other services as may be needed from time to time per the Department for Public Health’s Core Clinical Service Guide.
Pay Range: Pay will be commensurate with education and relevant experience but is anticipated to be in the range of $22-$33 per hour.
Minimum Education, Training, or Experience: Associate degree in Nursing with licensure in Kentucky or a compact state. Experience with early childhood education/home health and/or diabetes and prediabetes preferred, but not required.
Requirement: Work will require travel to family homes or within the community setting when not in the agency office.
Additional requirements: Applicants and employees in this classification will be required to submit to a drug screening test and background check according to the agency’s policies.
